From this page you can access bSpace to access all of our forms and publications frequently used here on campus. These documents are available in both the "Word" (.doc) and "Acrobat" (.pdf) formats. If you use the Acrobat forms you can type into them online then print them out for a tidy document. To download these forms, click on the link pertaining the form you want and either "open" it or "save it to disk" for your future reference.

If you need instructions on how to place orders, please activate this link or use the Jump menu and select "Placing Orders," above. For your convienience we have pre-entered the BFS Account number on the forms pertaining to the service you are soliciting. Be sure to provide your email address if you want confirmation of your order/service.

NOTICE: To access bSpace you will need a CalNet ID. Each department has a "CalNet" deputy who help you obtain a CalNet Id. We apologize but non-campus affiliates are NOT eligible for a CalNetID. (Campus Deputies List).

Here's a quick synopsis:

1.) After you've obtained your CalNet ID please access bSPACE.berkeley.edu

2.) After a successful login into bSpace, please click on "Membership"; this is a left-hand menu option on your screen.

3.) Please click on "Joinable Sites."

4.) In the search box enter: "OLAC Forms & Publications" and then click "Search"
***(Please note: Some users have reported after doing this, they web-site displays "site not found" this occurs when one searches for "OLAC" only rather than the whole name of the site ("OLAC Forms & Publications") if this is the case please ignore the sentence and please click on the link.)***

5.) Upon finding the OLAC Forms & Publications site, please "Join" it (by clicking on the provided link of the same name).

6.) A new tab will show in your workspace (named: OLAC Forms & Pubs), please click on it.

7.) In the left-hand menu within the OLAC Forms & Publications work-space, you are given the option to click on "Resources" - this is the folder where our forms & publications are kept.
